Confusion inflicts a -2 accuracy penalty. Completely separate from other debuffs. If a Confused pokemon misses an attack or its dice pool is reduced to 0, they take 1 damage. There are two ways to cure Confusion without an item. One: switch. Two: spend an action to do Insight checks. Accumulate 5+ successes across all the checks, and you snap out of Confusion.
Skull Ruins uses the Confusion rules from a prior version of the game, so it works very differently there. (Roll Insight -2 at the start of the round. If the check fails and the pokemon misses an attack or its dice pool is reduced to 0 in the round, they may hurt themselves at GM discretion. Switching cures it, but it also wears off on its own after a few rounds)
Regardless of the iteration, it's REALLY dangerous.
